SHAPE Arkansas Spring Break Adventure Challenge Waiver and Release of Liability
By signing or checking this box, I acknowledge the following:
1. Voluntary Participation
I understand that participation in the SHAPE Arkansas Presidential Fitness Challenge is completely voluntary and involves physical activity, including but not limited to running, walking, strength, endurance, and flexibility challenges.
2. Health & Safety
I certify that I/my child is physically able to participate in this fitness event and has no known condition that would prevent safe participation. I agree to ensure that proper precautions are taken, including warm-up, hydration, and monitoring intensity levels.
3. Risk Acknowledgement
I understand that all physical activity carries some risk of injury or accident, including but not limited to muscle strain, falls, or fatigue. I assume all such risks and take full responsibility for my/my child’s participation.
4. Release of Liability
I, for myself and on behalf of my child (if under 18), hereby release and hold harmless SHAPE Arkansas, its staff, volunteers, board members, and all affiliated individuals or entities from any and all liability, claims, demands, or causes of action arising out of or related to participation in this event.
5. Photo & Score Consent (Optional)
☐ I give permission for my/my child’s name, performance, and/or image (photo or video) to be shared in public recognition by SHAPE Arkansas on its website, social media, newsletters, and related publications.☐ I do not give permission for public recognition.
